In the UK a Radiator installation will have hot water flowing through it from the lower holes at each side. This is usually through a Valve to regulate the heat then out through a lock-shield.
The two holes at the top will have an Air Vent one side and Blanking Plug at the other. This allows a choice of whichever side is more convenient. Blanking Plugs are mostly found on Column Radiators whilst Cast Iron Radiators have Bushes each side, one of which will accommodate an Air Vent.
